|Amount||One award of $1,000 is available.|
Awarded annually to a second year CSD student seeking career opportunities to serve individuals with communication disorders in a medical rehab setting. The recipient must be in good academic standing with a minimum GPA of 3.7.
Students who are Mayo Clinic employees or considering becoming employees of Mayo Clinic may receive special consideration.
Financial need also will be a consideration in the selection.
|Application Procedure||Only applicants who complete the scholarship application form and have at least two letters of recommendation for the scholarship submitted by February 1 will be eligible for the award. Application/nomination forms are available from the CSD Graduate Coordinator.|
|Application Deadline||October 1 of the student's second year of the graduate program.|
|Selection Procedure||Determined based upon a combination of past accomplishments and strong undergraduate academic records. A committee of past CSD master's degree graduates will independently rank applications for this scholarship. In the event of a tie, committee members will be asked to rank the tied applications. The Scholarship Committee will submit its rankings to the CSD Graduate Coordinator.|
|Notification||The winner will be notified in writing. The chair of CSD will submit a scholarship award form to the Financial Aid office. The scholarship award will be applied to the recipient's account with the Business Office for application toward tuition and fees.|
|Special Notes||The recipient is required to send a thank you note within 30 days of receiving notification of the award to the Mayo Clinic Health System - Eau Claire Foundation, Inc., in care of the UW-Eau Claire Foundation. Submit the letter to the CSD Graduate Coordinator, who will in turn forward the letter on to the Foundation.|
